For the payment of the rate for the authorization of outlets with surcharges, proceed as follows:

1. Download the model.
2. Read the instructions at the end of the model
3. Complete electronically in the form. Not to be accepted official forms completed by hand or photocopies.
4. Print all pages, each of them in a one-sided.
5. The supply Expendedor must sign all copies.
6. The applicant (holder of the establishment) must sign all the copies as passive subjects.
7. Go to the bank with the model printed complete and signed, and make the payment.
   1. See the entities to make payment by direct debit.
   2. See the entities to make the payment with credit card.
8. Once payment is made, the bank will retain a copy and give you the other 3 copies: a copy for the applicant, a copy to the Expendedor supply and a copy to the Commissioner for the tobacco Market:
   1. Copy to the commissioner for the tobacco Market: once stamped by the bank, certification according to the next paragraph.
   2. Copy to the Expendedor: give it to the Expendedor.
   3. Copy to the applicant: once stamped by the bank, store as proof of payment.
9. In the case of kiosks or convenience stores, attach the following:
   1. Kiosk: administrative title, necessary the occupation of municipal public domain.
   2. Convenience store not located in service station: supporting certification issued by the competent authority in the field of trade.
10. Send us the copy of the commissioner for the tobacco Market accompanied by the relevant documentation in the case of kiosks or convenience stores, via one of the following methods
    1. Electronic procedure PVR devices.
    2. Registration office of any Administration.
    3. Postal mail, to the commissioner to the market of tobacco, Paseo de la Habana, 140, 28070, Madrid.
11. Once qualified, you will be notified in electronic form in Citizen Folder or in his home by postal mail.
